Advance Care Planning

Advance Care Planning is an important part of life planning. It is about your wishes, values and preferences for your future care if you can no longer speak for yourself. Who will do this for you? Have you designated a Substitute Decision Maker?
On Advance Care Planning Day, Canadians are encouraged to talk openly with family, friends and loved ones. Tell them what would be most important to you if you couldn’t speak for yourself. Tell them what brings you joy. Above all, tell them about your values and wishes.

Many hospice societies within the Federation provide support and education around Advance Care Planning.
Check with the hospice society in your community for workshops or help with your plan.
